This list contains some of the most frequently need forms for Scouts and parents.

BSA Youth Application This application is required for Scout membership in Boy Scouts of America and Troop 555. Please submit two copies to our Troop Committee Chair along with your dues.
BSA Health Form
Parts A,B (Combined)
Parts A&B of the Health Form are to be completed at least annually by participants in all Scouting events. This health history, parental/guardian informed consent and release agreement, and talent release statement are to be completed by the participant and parents/guardians. Please see the Annual Health and Medical Records page on the BSA website for more details.
BSA Health Form
Parts A,B,C (Combined)
Part C of the Health Form is the physical exam that is required for participants in any event that exceeds 72 consecutive hours, for all high-adventure base participants, or when the nature of the activity is strenuous and demanding. Service projects or work weekends may fit this description. Part C is to be completed and signed by a certified and licensed heath-care provider—physician (MD or DO), nurse practitioner, or physician assistant. It is important to note that the height/weight limits must be strictly adhered to when the event will take the unit more than 30 minutes away from an emergency vehicle, accessible roadway, or when the program requires it, such as backpacking trips, high-adventure activities, and conservation projects in remote areas. BSA Health Form
Part D
Part D of the Health Form is required to be reviewed by all participants of a high-adventure program at one of the national high-adventure bases and shared with the examining health-care provider before completing Part C. BSA Adult Application Quality leadership is important in the training of youth as members of the Boy Scouts of America. This application helps select the best individuals for their volunteer roles. Please submit two copies to our Troop Committee Chair.
UA Troop 555 Resource Survey This survey is used to identify opportunities for parents to support the troop. If you are interested in becoming more involved in the troop, please submit a form to the Troop Committee Chair.
BSA Merit Badge Counselor
Application
Parents who wish to become Merit Badge Counselors are required to submit this form to the Troop Advancement Chair. A BSA Adult Application must also be on file.
